What Is Hepatitis B Reactivation? Label Warning Explained

Hepatitis B Reactivation · say it: hep-uh-TY-tis BEE ree-ak-tih-VAY-shunAlso written: HBV reactivation, hepatitis B flare, reactivation of hepatitis B

Short answer

Hepatitis B reactivation is when a past or quiet hepatitis B infection becomes active again, often due to medicines that weaken the immune system.

What it means

Hepatitis B is a virus that infects the liver. In some people the virus stays in the body in a quiet or controlled state for years, even after they seem to have recovered. Hepatitis B reactivation means the virus starts multiplying again. This can cause liver inflammation, which may be mild, but in some cases it can lead to serious liver damage or liver failure. Yellow skin or eyes, dark urine, severe tiredness or tummy pain need prompt medical attention.

Labels carry this warning, sometimes as a boxed warning, for medicines that weaken the immune system, such as rituximab and some other cancer or autoimmune treatments, and for some antiviral medicines for hepatitis C or HIV. The label asks for a hepatitis B blood test before starting treatment, so that people at risk can be monitored or given medicine to prevent reactivation.

Reactivation is different from catching hepatitis B for the first time. A past infection can show up on blood tests (such as hepatitis B core antibody) even if you never knew you had it.

Where you may see it

  • Boxed warnings on some immune-suppressing and antiviral labels
  • Hepatitis B blood test results (HBsAg, anti-HBc)
  • Oncology and rheumatology pre-treatment checklists

📄 How it looks on a report

Screen all patients for HBV infection by measuring HBsAg and anti-HBc before initiating treatment.

🙅 What it does not mean

  • A screening test does not mean your doctor thinks you have hepatitis B; it is routine for these medicines.
  • A past infection that has 'cleared' can still reactivate with some treatments.
  • Reactivation can often be prevented or managed when it is detected early.
