What Is Hypogonadism? Low Testosterone Explained
Hypogonadism · say it: hy-poh-GOH-nad-iz-umAlso written: low testosterone, low T, testosterone deficiency, hypogonadotropic hypogonadism, primary hypogonadism
Short answer
Hypogonadism means the testes or ovaries make too little of their sex hormones, such as testosterone or estrogen.
What it means
Hypogonadism means the sex glands (gonads), which are the testes in males and the ovaries in females, produce too little of their hormones. In males this usually means low testosterone, which can cause low sex drive, erection problems, tiredness, loss of muscle and thinning bones. 'Primary' hypogonadism means the problem is in the gonads; 'secondary' or 'hypogonadotropic' means the brain's signals to the gonads are reduced.
Testosterone medicine labels list hypogonadism under 'Indications' and say it should be confirmed by symptoms and blood tests, not by age alone. Their warnings cover raised red blood cell counts, effects on the prostate, blood clots, possible changes in blood pressure, and harm if children or pregnant people are exposed to testosterone gel on someone's skin. Other medicines, such as long-term opioids, list hypogonadism as a possible side effect.
Hypogonadism is not the same as normal hormone changes with ageing, though they can overlap. In females, menopause is the natural end of ovarian hormone production and is usually not called hypogonadism.

📄 How it looks on a report
Indicated for testosterone replacement therapy in males with conditions associated with a deficiency or absence of endogenous testosterone.
🙅 What it does not mean
- One low testosterone result does not confirm hypogonadism. Testing is usually repeated, often in the morning.
- Hypogonadism is not just about sex drive; it can affect bones, mood and energy.
- Testosterone treatment is not suitable for everyone with low energy; the cause needs to be found first.
