What Is Paradoxical Bronchospasm? An Inhaler Warning

Paradoxical Bronchospasm · say it: par-uh-DOK-sih-kul BRONG-koh-spaz-umAlso written: paradoxical airway narrowing, inhaler-induced bronchospasm

Short answer

Paradoxical bronchospasm is a sudden tightening of the airways right after using an inhaler meant to open them; it needs urgent medical help.

What it means

Paradoxical bronchospasm is when the airways suddenly tighten straight after using an inhaled medicine, instead of opening up. Symptoms include sudden wheezing, coughing, chest tightness or breathlessness that is worse just after a puff. It needs urgent medical help, and the inhaler involved should be checked with a doctor before it is used again.

'Paradoxical' means the opposite of what is expected. Inhaler labels list it because almost any inhaled medicine, including reliever inhalers, can rarely trigger this reaction, possibly from the medicine itself or other ingredients. It is uncommon, but because it can be serious, labels make sure people recognise it.

It is different from ordinary bronchospasm caused by asthma triggers such as cold air, exercise or allergens, and different from a cough or throat irritation caused by inhaler technique.

📄 How it looks on a report

Paradoxical bronchospasm: If it occurs, discontinue the inhaler immediately and institute alternative therapy.

🙅 What it does not mean

  • A mild cough after an inhaler is not automatically paradoxical bronchospasm; the key sign is sudden worsening of breathing.
  • Being listed on a label does not mean it is likely to happen to you; it is uncommon.
  • It does not mean all inhalers are unsafe for you; a doctor can help choose a different one.
