What Are Vital Signs?
Vital signs · say it: VY-tul SYNZAlso written: vitals, observations, obs, obs chart
Short answer
Vital signs are basic body measurements, such as heart rate, blood pressure, breathing rate, temperature and oxygen level.
What it means
Vital signs are simple measurements that give a quick picture of how the body is working. The main ones are pulse (heart rate), blood pressure, breathing (respiratory) rate, temperature and oxygen saturation.
Hospitals check them regularly and often record them on an "obs chart" or in a table in your notes. Writing them down lets staff see how you are doing over time, and some hospitals use scoring systems that add up the readings to help spot when someone needs closer attention.
Vital signs shift through the day and with activity, pain, worry, fever and medicines. Doctors look at the pattern over hours or days as well as at single readings.
Where you may see it
- Hospital observation charts at the end of the bed or in the electronic record
- The triage section of emergency department records
- The 'On examination' part of clinic letters and discharge summaries
- Pre-operative checklists and anaesthetic records
📄 How it looks on a report
Obs: HR 88, BP 128/76, RR 16, Temp 37.0, SpO2 97% on air.
🙅 What it does not mean
- One unusual reading does not necessarily mean something is wrong. A single reading can be affected by moving, talking, pain or nerves, so staff often repeat it.
- Normal vital signs do not rule out every illness. They are a quick overview, not a full check of every organ, which is why blood tests or scans may still be ordered.
- "On air" means breathing normal room air. It does not mean you were given extra oxygen.
