What Is Cryptococcal Meningitis? Plain-English Guide

Cryptococcal meningitis · say it: krip-toh-KOK-ul men-in-JY-tisAlso written: cryptococcal meningoencephalitis, Cryptococcus infection of the brain, CM

Short answer

Cryptococcal meningitis is a serious fungal infection of the lining around the brain, mostly in people with weakened immune systems.

What it means

Cryptococcal meningitis is an infection of the meninges, the protective layers around the brain and spinal cord, caused by a fungus called Cryptococcus. The fungus is found in the environment, for example in soil and bird droppings, and is breathed in. It mainly causes illness in people whose immune systems are weakened, such as those with advanced HIV or on strong immune-suppressing medicines. It is a medical emergency: a worsening headache, fever, stiff neck, confusion, vomiting or vision changes need urgent medical help.

It appears on antifungal medicine labels, such as fluconazole and amphotericin B, under 'Indications', because these are used to treat or prevent it. It also comes up in HIV medicine warnings about immune reconstitution syndrome, because an unrecognised cryptococcal infection can flare when the immune system recovers.

Cryptococcal meningitis is different from bacterial or viral meningitis, which have different causes and treatments. A lumbar puncture (spinal tap) and tests such as the cryptococcal antigen test help tell them apart.

🙅 What it does not mean

  • It is not contagious from person to person in normal contact.
  • A positive cryptococcal antigen blood test does not always mean meningitis; further tests decide that.
  • It is not the same as bacterial meningitis, although both are serious.
