What Is Intracranial Hypertension? The Medicine Warning

Intracranial hypertension · say it: in-truh-KRAY-nee-ul hy-per-TEN-shunAlso written: raised intracranial pressure, idiopathic intracranial hypertension, IIH, pseudotumor cerebri, benign intracranial hypertension

Short answer

Intracranial hypertension means raised pressure of the fluid around the brain, which can cause headaches and vision problems.

What it means

Intracranial hypertension means the pressure inside the skull is higher than it should be, usually because of a build-up of the clear fluid that surrounds the brain and spinal cord. Typical symptoms are a headache that is often worse lying down, blurred or double vision, brief greying-out of vision, and a whooshing sound in the ears. A severe headache with vision changes needs prompt medical assessment, because untreated raised pressure can damage the nerve at the back of the eye.

Some medicine labels, such as those for tetracycline-class antibiotics and some vitamin A-related medicines, list it because these medicines have been linked to it in rare cases. The warning is there so that a new, persistent headache or change in vision is reported quickly rather than put down to stress or tiredness.

It is not the same as high blood pressure (hypertension), which is the pressure in the arteries. 'Idiopathic intracranial hypertension' means raised pressure with no clear cause, and 'benign' in older names does not mean it can be ignored.

Where you may see it

  • The 'Warnings and Precautions' section of doxycycline and minocycline labels
  • Leaflets for some acne and vitamin A-related medicines
  • Neurology and eye clinic letters

📄 How it looks on a report

Intracranial hypertension (pseudotumor cerebri) has been associated with the use of tetracyclines.

🙅 What it does not mean

  • It is not the same as high blood pressure. It is about fluid pressure around the brain.
  • Being on the label does not mean it is likely. It is a rare effect that needs to be recognised early.
  • The old name 'benign intracranial hypertension' does not mean it is harmless; vision can be affected if it is not treated.
